If You Require a Form to Be Filed in District Court:

     A person is always welcome to visit the District Court Clerk to obtain the requisite forms to start their respective lawsuit.  One may also follow the following link to find the appropriate paperwork to begin Landlord/Tenant, Small Claims, Civil Cases, etc.  Although many of these forms can be found online, one must still come into the District Court to officially file and serve the defendant.
